Lifestyle Behaviour Change Referral Service

Vision Health provides a structured lifestyle behaviour change service designed to support NHS Primary Care, General Practice and other healthcare providers in helping patients improve their long-term health outcomes.

We recognise that sustainable improvements in health rarely result from a single consultation. Lasting behaviour change requires personalised support, regular monitoring, professional coaching and measurable progress over time.

Our referral service complements existing NHS care by providing patients with ongoing health coaching, structured exercise support and digital monitoring between clinical appointments.

Our Service Includes

Each referral programme is tailored to the individual's needs and may include:

  • Dedicated Vision Health coaching team

  • Initial health screening and baseline assessment

  • Health measurements including:

    • Body Mass Index (BMI)

    • Blood pressure

    • Resting heart rate

    • Waist-to-hip ratio

  • Functional assessments including:

Flexible Referral Pathways

Referral programmes are designed around clinical need and can be adapted for different patient groups and referral pathways.

Whilst programmes can vary in duration, we typically recommend a minimum five-week intervention incorporating at least five coaching or personal trainer sessions to allow sufficient time for meaningful behaviour change and measurable progress.

Longer programmes can be delivered where clinically appropriate.

Measuring Outcomes

Throughout the programme, patient progress is monitored using objective health measurements together with functional and lifestyle assessments.

Where appropriate, referring clinicians receive summary outcome reports detailing baseline measures, progress throughout the intervention and end-of-programme results.

Real-Time Service Transparency

Vision Health provides secure reporting dashboards for authorised NHS users.

Referring organisations can monitor referral activity, patient engagement, programme completion and outcome measures through the Vision Health Portal, supporting governance, service evaluation and continuous improvement.

Medical-Grade Full Body Composition Analysis

For organisations requiring more detailed health assessments, Vision Health offers optional medical-grade body composition analysis using InBody technology.

Unlike traditional weight or BMI measurements, InBody provides a comprehensive picture of an individual's body composition, helping coaches and healthcare professionals monitor meaningful changes throughout the programme.

Assessments may include:

  • Body composition analysis

  • Skeletal muscle mass

  • Body fat percentage
